What Is the Sciatic Nerve?

The sciatic nerve begins in the lower back as five separate nerve roots that extend from five different vertebrae: L4, L5, S1, S2, and S3. These nerves meet up to form one large bundle—the longest and widest nerve in the human body—which runs through the buttocks and down the entire length of the leg.

The sciatic nerve is responsible (directly or indirectly) for nearly all the sensation in your leg, including the skin of the thigh, calf, and glutes.

What Causes Sciatic Nerve Pain?

Sciatica is defined as pain or irritation along the path of the sciatic nerve. It is important to remember that sciatica is a symptom (like a cough or an itch), not an independent diagnosis (like Parkinson’s disease).

Sciatica can range from an occasional nuisance to excruciating pain that makes walking nearly impossible. While it is most often associated with shooting pain, other symptoms may include:

  • Tingling or “pins and needles”

  • Numbness in the leg or foot

  • Muscle weakness in the affected leg

The L5 Connection: Alignment vs. Wear and Tear

In the lumbar spine, the L5 disc is the most commonly damaged disc in the entire back. One of the largest nerve pathways in the body passes directly through the narrow opening between the L5 vertebra and the sacrum. This critical pathway travels down your legs and controls vital functions, including:

  • Lower extremity strength (controlling the muscles in your legs and feet)

  • Digestive and bowel tract health (large intestines)

  • Reproductive and pelvic health (including the gonads and sexual function)

When looking at digital X-rays, most people suffering from sciatica show a significantly decreased space for this nerve to travel through. Why does this space shrink?

The Car Tire Analogy

Think of your spine like the wheel alignment on your car. If your front end is misaligned, your tires will wear down unevenly and balding will happen much faster.
